Schwindel ist eines der häufigsten Beschwerdebilder in der ärzt- lichen Praxis überhaupt. Er findet sich in vielen Fachdisziplinen wie der Oto-Rhino-Laryngologie, der Neurologie, der Ophthalmolo- gie, der inneren Medizin und auch in der Psychiatrie. Dieser Pro- blemkreis stellt also eine interdisziplinäre, diagnostische und thera- peutische Herausforderung dar. In den letzten Jahren sind zahlreiche Bücher zur Diagnostik des Schwindels erschienen, jedoch nur sehr wenige zur Therapie. Dies deutet darauf hin, daß auf dem Gebiet der Diagnostik mehr Sicher- heit herrscht als in der Behandlung. Hier soll der Versuch gemacht werden, eine Behandlungsmethode vorzustellen, die auf neueren Erkenntnissen der Pathophysiologie des vestibulären Systems auf- baut. Einem kurzen Überblick über die diagnostischen Verfahren bei Schwindelbeschwerden folgt eine knappe Darstellung der Physiolo- gie des vestibulären Systems. Daran anschließend werden patho- physiologische Mechanismen für die Schwindelentstehung darge- stellt. Eigene tierexperimentelle Untersuchungen, die zusammen mit J. Lannou in Paris und Rouen durchgeführt wurden, bilden zusammen mit Befunden anderer Autoren die Grundlage für ein modernes Konzept der vestibulären Kompensation. Daraus lassen sich Überlegungen zur therapeutischen Beeinflussung dieser Erho- lungsvorgänge ableiten. Das an der HNO-Klinik des Klinikums rechts der Isar verwandte Trainingsprogramm stützt sich auf Vor- schläge aus Großbritannien und Frankreich und hat diese modifi- ziert. Die hier berichteten Erfahrungen stammen von 50 Patienten mit peripher-vestibulären Erkrankungen. Den Abschluß des Buches bilden Überlegungen zur Pharmakotherapie, die sich gleich- falls auf das heute gültige Konzept der vestibulären Kompensation stützen. Untermauert wird dies durch eigene Erfahrungen mit einer nicht-sedierenden Substanz.
