El procedimiento mediante el cual un niño es alimentado con leche materna humana se conoce como lactancia o amamantamiento. La leche materna puede provenir directamente de la madre o puede extraerse y dársela al niño. La lactancia materna debe comenzar durante la primera hora de vida del recién nacido y debe continuar con la frecuencia y cantidad que el bebé desee, según la Organización Mundial de la Salud (OMS). La OMS y otras organizaciones sanitarias aconsejan la lactancia materna exclusiva durante seis meses. Aparte de la vitamina D, normalmente no se administran otros alimentos ni bebidas. La OMS aconseja amamantar exclusivamente durante los primeros seis meses de vida y luego continuar con la adición de alimentos complementarios adecuados hasta los dos años y más. Sólo el 42% de los 135 millones de bebés que nacen cada año son amamantados exclusivamente durante la primera hora de vida, sólo el 38% de las madres lo hacen durante los primeros seis meses y sólo el 58% de las mujeres continúan amamantando a sus hijos después de los dos años. La fórmula infantil carece de una serie de ventajas que la lactancia materna tiene tanto para la madre como para el niño. En los países de ingresos bajos y medios, las tasas de lactancia materna pueden aumentar a niveles casi universales, evitando aproximadamente 820.000 muertes de niños menores de cinco años cada año. Tanto en los países pobres como en los ricos, la lactancia materna reduce el riesgo del bebé de sufrir infecciones del tracto respiratorio, infecciones de oído, síndrome de muerte súbita del lactante (SMSL) y diarrea. También se han sugerido como ventajas adicionales menores posibilidades de desarrollar diabetes, alergias alimentarias y asma. Además, la lactancia materna puede mejorar el crecimiento cognitivo y reducir el riesgo de obesidad en adultos. Una menor pérdida de sangre después del parto, mejores contracciones uterinas y una menor probabilidad de depresión posparto son ventajas para la madre. La amenorrea de la lactancia, una condición donde la lactancia materna retrasa el regreso de la menstruación, también puede afectar la fertilidad en situaciones muy particulares. La reducción del riesgo de cáncer de mama, enfermedades cardiovasculares, diabetes, síndrome metabólico y artritis reumatoide son algunas de las ventajas a largo plazo para la madre.
