Ante el terrible asesinato de una prostituta llamada Inés de las Muñecas, Julián, un periodista y escritor que frecuenta los cafés de la ciudad de San Lorenzo no solo por gusto, sino porque está escribiendo un reportaje sobre estos establecimientos y las virtudes del café, decide hacer un experimento para comprobar ciertas teorías sobre el comportamiento de los marginados social y económicamente. Junto al reguero de sangre que queda en la esquina donde Inés fue asesinada, tras el levantamiento del cadáver, Julián decide poner una vela cada noche y difundir el rumor de que Inés, la prostituta santa, hace milagros a quienes creen en ella. El efecto es tal que estos milagros comienzan a suceder, según relatan los personajes a lo largo del libro.

"Santo Vituperio" es una novela coral en la que los personajes principales y secundarios se convierten en narradores, aportando sus puntos de vista sobre el asesinato, los posibles orígenes de la víctima y los pormenores de su posible canonización. Incluso tenemos la versión de la propia Inés, que nos cuenta su historia antes de ser asesinada. A menudo, estas narraciones son contradictorias; sin embargo, un narrador invisible guía a los lectores para que vayan atando cabos, demostrando la gran maestría de Homero Carvalho.

El papel de los intelectuales, que se agrupan en torno a las tertulias de los cafés y representan en su mayoría a una clase media liberal con sus contradicciones y aciertos, es destacable. También son importantes las noticias de los periódicos, que cuentan los acontecimientos de la región y proporcionan un contexto histórico. Además, está el papel de la Iglesia Católica, representada por el sacerdote Pedro María Almanza, a quien se le asigna la tarea de entrevistar a numerosos testigos, incluidos miembros de las clases bajas y devotos de la santa de la calle. Por encargo de sus superiores, intenta desvelar la mentira de los milagros de Inés, con la intención de ganarse a las congregaciones católicas y tradicionalistas de San Lorenzo. Sin embargo, cuando el padre Pedro cree haber desvelado los secretos de Inés de las Muñecas, sucede algo inesperado que afecta profundamente la conciencia del sacerdote.

El principal protagonista es, en última instancia, toda la ciudad de San Lorenzo, de ahí el carácter coral o polifónico de la novela.
