Methodische Fortschritte in der Liquordiagnostik zwingen auch zu neuen Einsichten uber die physiko-chemischen Voraussetzungen dia gnostischer Verfahren und zu differenzierteren Interpretationen der Befunde in Relation zur klinischen Situation. Die Analyse der Liquor proteine nimmt dabei eine zentrale Stellung ein. Die bis vor etwa 40 Jahren nur grobe Unterteilung in Albumin und Globulin erfuhr mit der Entwicklung elektrophoretischer Methoden eine entscheidende Verfei nerung, die zu einer immer praziseren Identifikation zahlreicher Pro teinkomponenten fuhrte. Dabei stand die Untersuchung des Immun globulin G im Mittelpunkt. Diese Monographie befasst sich mit den grundlegenden Aspekten des Immunglobulin G bei der multiplen Sklerose. Hinsichtlich der Metho dik sind dabei wesentlich: erstens die exakte, simultane, quantitative Bestimmung des Proteins und seiner Komponenten im Liquor und im Blutserum und die Errechnung der intrathekalen Immunglobulin-G Produktion unter Berucksichtigung der Bluthirnschrankenfunktion und zweitens die differenzierte Analyse der oligoklonalen Banden im IgG Bereich mittels der isoelektrischen Fokussierung. Die Information, die sich im Hinblick auf Diagnose und Verlauf der Erkrankung ergibt, wird dargestellt und kritisch bewertet. Das Ergebnis der Studie von Herrn Schipper ist eine informative Darlegung derzeitiger Erkenntnisse uber das Immunglobulin G, die durch die Liquoruntersuchung am Krankenbett ermittelt werden kon nen und fur alle weiteren differenzierteren biochemischen und immu nologischen Befunde unverzichtbares Basiswissen darstellen.
