Der Diabetes mellitus ist eine Volkskrankheit. Bei seiner Diagnose im Erwachsenenalter sind häufig schon erste Schäden an Gefäßen und Nerven feststellbar. Die Pathogenese der diabetischen Folgeerkrankungen ist trotz intensiver Forschungen noch nicht umfassend geklärt. Außer Zweifel steht jedoch, dass der erhöhte Blutglucosespiegel, die Hyperglycämie, verbunden mit oxidativem Stress eine entscheidende Rolle spielt. Die Behandlung des Diabetes mellitus und seiner Folgen stellen hohe Anforderungen an das Gesundheitswesen. Durch Vermeidung oder Minderung der Folgen einer chronischen Hyperglycämie können die Lebensqualität der Erkrankten verbessert und erhebliche Kostenreduktionen erreicht werden. Eine Ursache der Diabetes-spezifischen Komplikationen ist die gesteigerte Maillard- Reaktion, d. h. die nichtenzymatische Umsetzung von Proteinen, Lipiden und Nucleinsäuren vor allem mit Glucose, die als Blutzucker die Hauptenergiequelle für alle Zellen darstellt. Die größten Schäden treten in Organen und Geweben mit einem hohen Collagengehalt sowie in Organen auf, deren Glucoseaufnahme nicht durch Insulin reguliert wird, wie Niere, Retina und das Endothel. Diese Befunde stützen die Glycierungshypothese als wesentlicher pathogenetischer Prozess diabetischer Folgeerkrankungen. Aber auch die Arteriosclerose, cardiovasculäre Erkrankungen als Folge einer chronischen Niereninsuffizienz, die Hämodialyse-assoziierte Amyloidose beim chronischen Nierenversagen und degenerative Hirnerkrankungen, wie der Morbus Alzheimer, weisen Beziehungen zur Maillard-Reaktion auf. Bedingt durch die demographische Entwicklung werden Diabetes, Arteriosclerose und degenerative Hirnerkrankungen in Ländern mit einer hohen Lebenserwartung weiter zunehmen. Ebenso zu berücksichtigen ist das physiologische Altern, welches ebenfalls auf einer Akkumulation von Glucose-Addukten beruht. Zellphysiologische Aspekte der Maillard-Reaktion sind gerade in den letzten Jahren in den Vordergrund dieser Forschungen getrete
